Collaborate with nursing leadership and interdisciplinary teams to assess resident needs.
Inquiry calls and tours for short term rehab and long term nursing care
Coordinate admission paperwork, authorizations, equipment, and services
Conduct facility tours and help grow census through exceptional customer service.
Qualifications
Valid Illinois LPN License
Strong clinical assessment skills
Previous Medicare admissions preferred
Long-term care or healthcare experience preferred
Ability to multitask
Must have the desire to work with and serve seniors and their families
