How you will contribute:
The Admissions Coordinator plays a critical role in facilitating timely and accurate hospice admissions by coordinating referrals, patient records, physician orders, and patient/family appointments. This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team.
How you will make an impact:
Admissions & Referral Coordination
- Receive and process referrals from Community Liaisons, physicians, patients, families, and community partners.
- Coordinate requests for patient records.
- Coordinate and schedule patient and family appointments for informed consent and RN assessment visits.
- Verify and upload patient records to EMR.
- Ensure timely notification of new referrals, admissions, and non-admissions to the assigned interdisciplinary team (IDT) and ancillary providers.
- Coordinate supply and DME provision to meet patient needs.
- Follow up on patients who have not been admitted and/or have been discharged.
Documentation
- Maintain and organize patient charts, ensuring all documentation is complete, signed, and dated.
- Transfer completed admission charts to the Team Coordinator in a timely manner.
Patient & Family Support
- Explain hospice services, benefits, and eligibility to patients, family members, and referral partners, as needed.
- Serve as a compassionate and knowledgeable point of contact during the admission process.
Team Communication & Collaboration
- Inform appropriate team members of scheduled admission appointments and updates.
- Communicate identified needs, issues, and potential solutions to the supervisor.
- Attend and participate in staff meetings, educational programs, and community events, as requested.
The Experience and Skills You'll Bring
- Active, unrestricted Registered Nurse License
- Be available at all times during agency operating hours or as needed
- The ability to make sound professional clinical judgment
- Excellent verbal and written communication skills
- Proficiency with EMR and data entry systems
- Hospice experience preferred
