Admissions Coordinator

The Admissions Coordinator manages referrals and coordinates the admissions process for Country Manor. This role requires professional, respectful communication with internal teams and external referral sources across in-person, phone, and electronic channels. Responsibilities include responding to inquiries from prospective residents, conducting pre-admission assessments in hospital settings, and serving as a liaison among physicians, residents/patients, referral partners, and Country Manor.

The Admissions Coordinator is also responsible for verifying insurance coverage for skilled nursing services, including eligibility, benefits, and deductibles, and ensuring all information is accurately documented and communicated to the appropriate parties. This position upholds and reflects Country Manor's Hometown Culture, values, and commitment to integrity in all interactions.

Schedule includes Monday - Friday during normal business hours.

Qualifications:

• Licensed Practical Nurse in the State of Minnesota
• Must be skilled in oral and written communication
• Ability to work effectively with other health care professionals
• Knowledge of MDS, RUGS levels, Medicare guidelines, and reimbursement in the LTC facility
• Knowledge of costs associated with different levels of care
• Knowledge of and skill in using personal computers and related software for word processing, spreadsheets and clinical software applications
• Skill of developing and maintaining positive working relationships with clients and other employees; ability to respond with a sense of urgency to clients' needs

Responsibilities:

• Serve as the primary contact for all service inquiries, referrals, discharges, and transfers.
• Coordinate admissions by collaborating with hospitals, referral sources, and internal care teams.
• Conduct pre-admission assessments, verify required documentation (insurance, physician orders, demographics), and support admission decisions.
• Manage facility census, track bed availability, and communicate openings daily.
• Facilitate a smooth admission process, including tours, meetings, and ongoing process improvements.
• Maintain relationships with referral sources and promote services through outreach and events.
• Ensure timely communication, professional representation, and compliance with policies and procedures.
• Participate in meetings, manage time effectively, and perform additional duties as assigned.
