Chatham Hills Subacute Care Center is seeking an experienced and motivated Admissions Director to join our leadership team. The ideal candidate will be responsible for overseeing the admissions process, developing referral relationships, and helping drive census growth while ensuring a smooth transition for residents and families.

This is an excellent opportunity for a dynamic healthcare professional with strong relationship-building and organizational skills to join a growing skilled nursing and rehabilitation center.

Responsibilities

• Manage and oversee all admissions activities for the facility
• Coordinate referrals, screenings, and admissions processes
• Develop and maintain strong relationships with hospitals, case managers, discharge planners, physicians, and community referral sources
• Conduct facility tours for prospective residents and families
• Collaborate with clinical and interdisciplinary teams to ensure appropriate admissions
• Track admissions data, referral activity, and census goals
• Ensure timely communication and follow-up with referral sources
• Assist with marketing and business development initiatives
• Maintain compliance with all federal, state, and facility policies and regulations
• Participate in community outreach and networking events

Qualifications

• Previous admissions, marketing, business development, or healthcare sales experience preferred
• Skilled nursing facility or rehabilitation center experience strongly preferred
• Excellent communication, customer service, and relationship-building skills
• Strong organizational and multitasking abilities
• Ability to work in a fast-paced environment
• Knowledge of insurance verification and healthcare admissions processes preferred
• Professional, motivated, and team-oriented
