    Job Description

    We are seeking a motivated and relationship-driven Admissions Director to lead the admissions process for our Skilled Nursing Facility. The Admissions Director is responsible for developing referral relationships, coordinating admissions, maintaining census goals, and ensuring a seamless transition for residents from hospitals and other healthcare settings into the facility.

    Responsibilities
    • Manage the admissions process from referral through resident admission.
    • Build and maintain strong relationships with hospitals, case managers, discharge planners, physicians, home health agencies, hospices, and other referral sources.
    • Evaluate referrals to ensure appropriate clinical and financial acceptance.
    • Coordinate with the interdisciplinary team to facilitate timely admissions.
    • Conduct facility tours for prospective residents and families.
    • Educate residents and families regarding services, insurance coverage, and the admission process.
    • Collaborate with the Business Office to verify insurance benefits and obtain required authorizations.
    • Monitor and report census trends, referral activity, conversion rates, and occupancy goals.
    • Participate in marketing and business development initiatives to increase referrals and census.
    • Maintain accurate admission records and documentation in compliance with facility policies and regulatory requirements.
    • Ensure a positive admission experience and excellent customer service for residents and families.

    Qualifications
    • Bachelor's degree in Healthcare Administration, Business, Marketing, Nursing, Social Work, or a related field preferred.
    • Minimum of 2–3 years of admissions, marketing, business development, or case management experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), Hospital, or Post-Acute Care setting.
    • Knowledge of Medicare, Medicaid, managed care, and insurance authorization processes.
    • Strong understanding of post-acute care services and SNF admission criteria.
    • Excellent communication, customer service, organizational, and relationship-building skills.
    • Valid driver's license and reliable transportation for community outreach, if required.
