Job Description:

Shift: Full-Time Days

Hours: Monday- Friday 8am-5pm. Rotation Call Required.

Location: Hinsdale Hospice Office with support to Field Staff and supervisory visits of Admissions Staff.

  • Oversees the recovery process, including follow-up with patients who are non-admitted, discharged, or have revoked the Hospice Medicare Benefit, and ensures tracking and monitoring of changes for future hospice needs.
  • Maintains knowledge and communicates policies and procedures required to comply with federal, state, and local guidelines, as well as Medicare, Medicaid, and insurance reimbursement requirements.
  • Provides overall direction of operations related to admissions and referrals.
  • Develops, revises, and implements schedules for admission nurses to ensure staff availability.
  • Reviews nursing documentation and charting to ensure it is timely, accurate, and complete, and communicates with the Health Information Management Department to update incomplete documentation.
  • Accompanies staff periodically to evaluate performance and knowledge, providing expertise and guidance to improve skills.
  • Participates in staff meetings, department meetings, in-services, and other related activities.
  • Recommends and implements the fiscal budget for the assigned department, monitors budget variances, and reports findings. Other duties as assigned.
  • Communicates with healthcare providers regarding referrals, including doctors, hospitals, assisted living facilities, and skilled nursing facilities. Interprets requirements to determine eligibility for unique or questionable cases.
  • Communicates with PCM teams to determine and coordinate staffing levels.
  • Oversees the referral process to ensure patient referral calls for hospice services are appropriately answered and processed.

  • Knowledge of documenting within an electronic medical record. [Required]
  • Knowledge of basic to intermediate application of Microsoft Office Suite including Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
  • Knowledge of third party reimbursement and cost containment. [Required]
  • Knowledge of basic supervisory practices and procedures. [Required]
  • Ability to conduct interviews of prospective employees. [Required]
  • Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]

Pay Range:

$73,179.07 - $129,306.76
