    POSITION OVERVIEW

    The Admissions/Medical Records Nurse (LPN, RN) is responsible for overseeing the admissions intake and medical records functions of the Nursing Department(s) by assessing new residents and reviewing the baseline care plans with the nursing leadership team.

    Key Responsibilities

    • Review Braden Scale results implementing preventative measures for high-risk residents prior to wound development.
    • Review high risk residents upon admission using fall risk assessment ensuring orders for preventative interventions are in place, care plan for interventions entered and CNA sheets/profiles are updated.
    • Complete nursing admission assessment and baseline care planning for new admissions.
    • Complete MDS section GG Functional assessment during the required timeframe.
    • Make written and oral reports/recommendations to the Assistant and Director of Nursing as necessary/required, concerning the operation of the nursing service department.
    • Verify census events are correct in Electronic Health Record.
    • Complete resident documentation in MatrixCare to ensure Medical Records are complete and accurate.
    • Provide patient care as needed and participate in campus on-call rotation based on campus schedule.

    Qualifications

    • Must have and maintain a current, valid state LPN or RN license
    • Current, valid CPR certification required

    Compensation will be determined based on the relevant license or certification held, as well as the candidate's years of experience.
