Job Summary

We are seeking a skilled and compassionate Admissions Nurse (RN) to join our team. This role plays a key part in welcoming new residents, coordinating admissions, and ensuring a seamless transition into our facility.

This position requires strong clinical assessment skills, attention to detail, and a resident-centered approach.

Responsibilities

  1. Conduct initial clinical assessments of new residents
  2. Develop individualized care plans based on assessment findings
  3. Coordinate with families, physicians, hospitals, and other healthcare providers
  4. Ensure all required documentation is completed accurately and timely
  5. Educate residents and families on services, policies, and care processes
  6. Collaborate with the interdisciplinary team for continuity of care
  7. Provide emotional support during the transition process
  8. Participate in quality improvement initiatives related to admissions

Qualifications

  1. Current Registered Nurse (RN) license in good standing
  2. 1–2 years of nursing experience preferred (long-term care or geriatrics a plus)
  3. Strong assessment and documentation skills
  4. Knowledge of state and federal nursing home regulations
  5. Excellent communication and interpersonal skills
  6. Ability to work independently and collaboratively
  7. Compassionate, patient-focused approach
