Position Overview
We are seeking a motivated and experienced Nurse Practitioner (ARNP) to provide on-site medical care to patients in long-term care and residential facilities. This is a mobile position ideal for a provider who enjoys autonomy, building relationships, and making a meaningful impact in the elderly population.


Responsibilities

•  Provide primary and preventative care to geriatric patients in ALF, ILF, and SNF settings 
•  Perform comprehensive assessments, routine follow-ups, and acute visits 
•  Manage chronic conditions (HTN, diabetes, dementia, etc.) 
•  Coordinate care with facility staff, specialists, and families 
•  Complete documentation in EMR in a timely manner 
•  rotating weekend call 
•  Participate in care planning and interdisciplinary team meetings 
•  Ensure compliance with all regulatory and clinical guidelines 

Qualifications

•  Active Florida ARNP license 
•  Board Certified (ANCC/AANP)
•  Medicare credentialed a plus  
•  Experience in geriatrics, internal medicine, or long-term care preferred 
•  Strong clinical assessment and diagnostic skills 
•  Ability to work independently and manage a flexible schedule 
•  Reliable transportation (travel between facilities required)
