Adult Respite Case Manager
Part‑Time • Weekday Schedule • Hope’s Promise Respite

Position Summary

Our Adult Case Manager builds strong relationships with families, supervises mentors, and oversees the full matching process for individuals receiving respite services in the community.  This role is ideal for someone who communicates clearly, stays organized, and thrives in a structured, people‑centered environment. This is not a group home setting.

Schedule
-20 hours per week
-Primarily daytime hours
-Flexibility to set consistent base schedule that works for you

What You’ll Do

  • Serve as the primary point of contact for assigned families
  • Conduct meet and greets in the family's home to provide information, guidance and assistance with any application documentation
  • Maintain consistent, compassionate communication with families
  • Supervise mentors through regular check‑ins, coaching, and support
  • Oversee matching for assigned adult participants to ensure safety, compatibility, and program alignment
  • Monitor match progress and address concerns promptly
  • Review documentation for accuracy and compliance
  • Maintain organized case notes and follow established workflows
  • Collaborate with leadership on training, match stability, and program improvements

Requirements

  • Familiarity with adult DDD system
  • Bachelor's degree preferred; high school diploma with experience can be substituted
  • Strong communication and relationship‑building skills
  • Previous supervisory experience preferred
  • Comfort with structured documentation and digital tools
  • Valid driver’s license and reliable transportation
