Program Strategy: Oversee daily operations for Adult Day Care and Habilitation, including generating monthly activity calendars that promote community integration.
Compliance Excellence: Ensure high-quality documentation in our EHR (Vision Works) and maintain HCBS and DIA standards.
Community Connection: Act as a key liaison for families, guardians, and community partners to ensure seamless service delivery.
Experience: Minimum of 2 years in HCBS programs with knowledge of Adult Day Services. Prior supervisory experience is preferred.
Education: Associate's Degree in Human Services or a related field (preferred).
Skills: Proficiency in computer software, ability to lift 40 lbs, and a talent for team collaboration.
Requirements: Valid driver's license, auto insurance, and the ability to obtain CPR/First Aid within 30 days.
