As an NP with us, you'll play a vital role in delivering comprehensive healthcare services, promoting wellness, and effectively managing chronic conditions among our residents.

Key Responsibilities:

• Conduct comprehensive initial assessments and routine evaluations of residents' health statuses.
• Craft personalized care plans to address the unique needs of each resident, emphasizing preventive care and health maintenance.
• Oversee and coordinate ongoing care for residents with chronic conditions like diabetes, hypertension, and heart disease.
• Prescribe appropriate medications and treatments, adhering to facility protocols and regulatory guidelines.
• Collaborate with a dedicated interdisciplinary team of physicians, nurses, therapists, and caregivers to ensure coordinated, holistic care.
• Educate and support residents and their families in health management, disease prevention, and lifestyle adaptations.
• Respond swiftly to acute medical needs and emergencies, ensuring timely intervention and stabilization.
• Maintain precise and current medical records/documentation for all residents.
• Engage in regular team meetings, case conferences, and continuing education to remain up-to-date with best practices and industry standards.
• Full or Part time opportunities available

Qualifications:

• Current unrestricted Nurse Practitioner license in New York.
• Proven proficiency in conducting comprehensive health assessments and managing chronic conditions.
• Exceptional interpersonal and communication skills, with the ability to build rapport with residents, families, and staff.
• A compassionate, patient-centered approach to care, with a strong commitment to enhancing the quality of life for elderly individuals.
• Capability to work both independently and collaboratively in a multidisciplinary team environment.

Location: Amsterdam, NY
Salary: $160,000 annually
