Position Summary

The Registered Nurse-Care Team supports Adult Primary Care providers and care teams by delivering direct and indirect nursing care in a health center setting. This role assists with desktop medicine activities, including clinical review of patient messages, refill and results workflows, care coordination, and follow-up needs in accordance with established protocols. The RN provides timely triage and nursing assessment for patients who arrive at the health center with urgent or unscheduled needs, prioritizing care based on clinical presentation, psychosocial factors, patient safety, and overall patient flow. The RN collaborates closely with providers, Medical Assistants, and other support staff to promote efficient team-based care, communicate changes in patient status, and ensure appropriate escalation of symptoms, diagnostic results, or changes in condition. This position requires sound clinical judgment, initiative, flexibility, and the ability to respond quickly and accurately to patient needs with minimal supervision.

Required Qualifications

• Graduate of an accredited school of Nursing with up to six (6) months Registered Nurse (RN) experience.
• Current Registered Nurse (RN) license within the state of California and BLS certified.
• Previous nursing experience in Emergency Room or Urgent Care experience.
• Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
• Actively participate in performance improvement and continuous quality improvement (QA/QI) activities.
• Comply with all organizational policies regarding ethical business practices. Communicate the mission, ethics and goals of the organization.
• Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
• Computer (PC) literate in general office functions such as MS Word, access, email and preparation of spreadsheets.
• Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
• Adhere to dress code, appearance is neat and clean.
• Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
• Assist and support the Center/Department/Program to meet standards of High Reliability.
• Fluency in additional languages preferred.
• Must be willing to successfully complete the credentialing process.

Essential Responsibilities

• Provide direct patient care, evaluate outcomes, consult with other specialists as required and adjust nursing care processes as indicated to ensure optimal patient care.
• Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Accurately assign triage categories and direct patients to appropriate level of care.
• Answer telephone calls professionally; refer patient problems to appropriate licensed provider; schedule appointments.
• Maintain accurate and continued nursing documentation, including patient history, conditions, treatments, responses, and assessment of changes.
• Inform patients of results and answer questions per established protocols.
• Review Inbasket for refills, refill meds per set protocols, and connect with pharmacy as necessary for refills.
• As a link between provider and pharmacy for questions on dosages/alternative meds, etc, answer patient questions regarding medication.
• Respond to portal messages that require a clinical response (when appropriate).
• Respond to and complete requests from Home Health Agencies per an established protocol and obtain provider signatures when required.
• Review patient forms with the provider, complete them (when appropriate), and obtain the provider's signature per established protocol.
• Treat patients and their families with respect and dignity.
• Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family-functions as a liaison between administration, patients, physicians, and other healthcare providers.
• Demonstrate knowledge of growth and development principles over the life span and the skills necessary to provide age-appropriate care to the patients served. Able to interpret data about the patient's status to identify each patient's age-specific needs and provide the care needed by the patient group.
• Interact professionally with patient and family and involve patient and family in forming the plan of care.
• Formulate a teaching plan based on identified learning needs and evaluate learning effectiveness; include family in teaching, as appropriate.
• Maintain current knowledge of medications and the correct administration based on the age of the patient clinical condition.
• Follow the seven (7) medication rights to reduce the potential for medication errors.
• Demonstrate knowledge of lab requirements requisitions.
• Maintain established Center policies and procedures, objectives, performance improvement program, safety, environmental, and infection prevention and control standards.
• Assist in cost containment through appropriate ordering and conserving of supplies and equipment.
• Cooperate with other staff to achieve outpatient clinic objectives and maintain good employee relations.
• Attend annual review and department in services, as scheduled; staff meetings as required.
• Support and maintain a culture of safety and quality.
• May assist Center Manager and Lead Clinician with training and supervision of medical assistants and other support staff.
• Demonstrate knowledge of healthcare plans.
• Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops; show responsibility for own professional practice and ongoing education.
• Perform other related duties as assigned or requested.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
