Now Hiring - Advanced Practice Nurse - Teaneck NJ, Oradell NJ

Salary Range $125,000 to $135,000 a year

he Advanced Practice Nurse will be responsible for, but are not limited to:

  • Conduct visits to patients in accordance with local, state and federal regulations.
  • Communicate visit findings to designated members of the interdisciplinary team.
  • Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
  • Consult with other members of interdisciplinary team to contribute to the patient''s plan of care.
  • Prepare and maintain accurate patient records, charts and documents.
  • Assist in development of clinical practice guidelines/standards in support of quality care as needed.
  • Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education:

Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

Professional Experience:

  • Must have a Nurse Practitioner''s license in the state of operations
  • Must have 1+ years experience working within Family Practice, General Practice, Geriatric care or related field,
  • Must have a valid driver''s license and a reliable source of transportation
  • Must be willing to travel
  • Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
  • Commitment to ongoing professional development and clinical excellence.
