    Position Title: Advance Practice Provider - 4th Trimester Clinic (Grant Funded)

JPS offers 18 residency and fellowship programs, including the largest family medicine residency in the nation.

    Job Description:

    The Women's and Infants service line at JPS is a robust multidisciplinary team of physicians and advanced practice professionals who are dedicated to meeting the complete health care needs of women in our community. We provide services and support for women at every stage of life, including but not limited to preventative health care, low and high risk prenatal/childbirth/postnatal care, menopause care, urogynecology, and gynecologic oncology evaluation and treatment. Our physicians provide 24-hour in-house inpatient hospital coverage and we deliver and average of 4,000 babies a year. Our Advanced Practice Provider group consists of trusted and experienced nurse practitioners and certified nurse midwives who are committed to providing excellent health care to women and to ensuring that every child has the best possible start in life. They work in concert with our Physicians on a daily basis and are highly regarded by our patients and their families for the confidence they engender and the relationships they foster through attentiveness, responsiveness, and meaningful communication. Advanced Practice Providers are a valued part of our healthcare team and are integral to maintaining our commitment to providing quality health care to the patients and families that we serve.

    Position Details Weekdays (10-hour day shift) Fourth-Trimester Outpatient Clinic:

    • A dedicated postpartum access point for follow-up, blood-pressure management, depression screening, contraception, lactation, and resource referral.
    • Expand inpatient and outpatient care navigation.

    Duties and Essential Job Functions:

    1. Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.

    2. Reviews patient medical charts to assist physicians with diagnosis, treatment and care management. 3. Make appropriate patient referrals as necessary.

    3. Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.

    4. Provides care education to patients and families.

    5. Provides evidence-based and compassionate care to all patients assigned to improve patient outcomes and increase patient satisfaction scores.

    6. Maintains a collaborative environment with providers, clinical staff, and support staff.

    7. Supports academic, educational and research endeavors.

    8. Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.

    9. Serves as a resource to the community. 11. Performs other job-related duties as assigned.
