The Department of Corrections is seeking a highly motivated and qualified Advanced Care Practitioner for the Stafford Creek Corrections Center located in Aberdeen, WA.

Under the supervision of the Facility Medical Director, this position supports the agency's mission to improve public safety by delivering timely, high-quality, person-centered care to the incarcerated population. The Advanced Care Practitioner (ACP) manages a defined patient panel using a population health approach that emphasizes prevention, chronic disease management, and continuity of care. The care philosophy is built on a foundation of "making better neighbors" by encouraging recovery and individualized patient health literacy development.

The ACP assesses, diagnoses, and treats a broad range of medical conditions while partnering with patients in a shared decision-making, promoting health education, and supporting sustainable behavior change. Working within an interdisciplinary team, the ACP provides coordinated, equitable care that respects patient dignity and autonomy, including delivering primary care in the clinic and potentially in the living unit (house calls), attending urgent care in collaboration with nursing staff, and providing medical coverage for the inpatient unit.

This role also contributes to the quality and effectiveness of health services through data-informed practice, collaboration with Health Services Managers, the Facility Medical Director, and Team Members, and participation in planning, resource stewardship, and continuous system improvements.

Duties

Provide Direct Patient Care & Panel Management:

  • Deliver accessible, continuous, and person-centered primary care to a defined patient panel, consistent with National Commission on Correctional Health Care standards and Patient-Centered Medical Home principles.
  • Provide care in accordance with the Washington DOC Health Plan, Formulary, and published WA DOC clinical protocols and guidelines.
  • Perform initial and ongoing health assessments, screenings and physical examinations; initiate and update PULHES coding and health classifications.
  • Assess, diagnose, and treat acute and chronic conditions using evidence-based practices, emphasizing prevention, chronic disease management, and health equity.
  • Provide urgent and emergency care in collaboration with nursing staff across clinical settings, ensuring timely access and appropriate triage.
  • Admit, manage, and provide ongoing care for patients in the inpatient/infirmary unit, ensuring safe transitions across levels of care.
  • Coordinate care across disciplines and settings, including collaboration with community providers to supports continuity of care during transitions (e.g. intake and reentry), recognize the role of corrections health as an integral partner in public health.
  • Order and interpret diagnostic testing and prescribe medications, including controlled substances and medications for opioid use disorder (MOUD), in alignment with clinical guidelines and formulary standards.
  • Perform minor procedures within scope of practice.
  • Recognize and respond to changes in patient condition, escalating and facilitating specialty referral when indicated.

Communication, Documentation & Care Coordination:

  • Maintain timely, accurate, and complete documentation in the health records in accordance with NCCHC standards, legal requirements and DOC policy.
  • Communicate effectively and respectfully with patients and interdisciplinary team members, supporting shared decision-making and patient engagement; this includes all verbal and written communications (Teams, email, kites, SMS messages).
  • Coordinate transitions of care, including off-site services and community-based providers, ensuring continuity, information exchange, and follow-up.
  • Respond to patient requests (kites) and institutional communications in a timely, professional and patient-centered manner.
  • Protect confidentiality of health information in accordance with HIPPA, state law, and DOC policy.

Quality Improvement & Population Health:

  • Participation in Coordinated Quality Improvement (CQIP) activities consistent with NCCHC requirements and WA DOC initiatives for continuous quality improvement.
  • Lead or contribute to at least one quality improvement annually, using data to improve clinical outcomes, access and patient experience.
  • Apply population health strategies, including panel management, risk satisfaction, and proactive clinical outcomes, access and patient experience.
  • Support implementation of evidence-based practices and PCMH-aligned workflows to improve system performance and care delivery.
  • Share openly with colleagues across the state in an effort to support their practice, collaborate on improving systems, and raise the status of corrections medicine. As possible, extend participation to organizations such as ACCP, NCCHC, NIC and ACA.

Team-Based Care, Delegation & Clinical Support:

  • Function as part of an interdisciplinary care team, supporting PCMH principles of team-based, coordinated care.
  • Delegate and assign care responsibilities appropriately, ensuring safe, effective, and efficient care delivery.
  • Provide clinical guidance, mentorship, and support to nursing and other staff, promoting a culture of safety, respect, and continuous learning.

Collaboration with Custody & Institutional Partners:

  • Collaborate with custody and facilitate partners to support a safe, secure and therapeutic environment consistent with NCCHC and WA DOC Health Services standards.
  • Provide clinical input related to patient health, safety, and institutional operations, including grievances and emergent situations.

Professional Responsibilities & Other Duties:

  • ACPs must continually maintain licensure, certifications and registrations in good standing as a foundational requirement of your position.
  • Participate in required training, clinical oversight, peer review, professional standards reviews, and on-call responsibilities.
  • Adhere to professional standards, the Washington DOC Health Plan, Formulary and Published Clinical Protocols and Guidelines.
  • Contribute to the respectful, person-centered culture that recognizes the dignity and humanity of those served and supports the role of correctional health within the broader public health system.

Qualifications

Required Qualifications:

  • Must possess a valid Washington State license to practice as a Registered Nurse AND a possession of a current designation as an Advanced Registered Nurse Practitioner AND current certification from an approved national program (American Nurse Association)

OR

  • Possess a valid Washington state licensure to practice as a Physician Assistant AND current certification from an approved national certification program (National Commission on Certification of Physician Assistants)

AND

  • Must hold an unrestricted license to practice as a midlevel practitioner and prescribe scheduled drugs
  • Initial and continuous unblemished/unrestricted licensure in the State of Washington as a Registered Nurse and Advanced Registered Nurse Practitioner OR Physician Assistant.
  • Initial and continuous unblemished/unrestricted certification by ANCC or AANP or NCCPA for PA
  • Current Drug Enforcement Administration (DEA) registration.
  • A National Provider Identification (NPI) number.
  • Current certification in American Heart Association version of Basic Life Support.

Desired Qualifications:

Insurance Benefits

Retirement and Deferred Compensation

State Employees are members of the Washington Public Employees Retirement System (PERS). New employees have the option of two employer contributed retirement programs. For additional information, check out the Department of Retirement Systems web site.

Employees also have the ability to participate in the Deferred Compensation Program (DCP). This is a supplemental retirement savings program (similar to an IRA) that allows you control over the amount of pre-tax salary dollars you defer as well as the flexibility to choose between multiple investment options.

Social Security

All state employees are covered by the federal Social Security and Medicare systems. The state and the employee pay an equal amount into the system.

Public Service Loan Forgiveness

If you are employed by a government or not-for-profit organization, and meet the qualifying criteria, you may be eligible to receive student loan forgiveness under the Public Service Loan Forgiveness Program.

Holidays

Full-time and part-time employees are entitled to paid holidays and one paid personal holiday per calendar year.

Note: Employees who are members of certain Unions may be entitled to additional personal leave day(s), please refer to position specific Collective Bargaining Agreements for more information.

Full-time employees who work full monthly schedules qualify for holiday compensation if they are employed before the holiday and are in pay status for at least 80 nonovertime hours during the month of the holiday; or for the entire work shift preceding the holiday.

Part-time employees who are in pay status during the month of the holiday qualify for the holiday on a pro-rata basis. Compensation for holidays (including personal holiday) will be proportionate to the number of hours in pay status in the month to that required for full-time employment, excluding all holiday hours. Pay status includes hours worked and time on paid leave.

Sick Leave

Full-time employees earn eight hours of sick leave per month. Overtime eligible employees who are in pay status for less than 80 hours per month, earn a monthly proportionate to the number of hours in pay status, in the month to that required for full-time employment. Overtime exempt employees who are in pay status for less than 80 hours per month do not earn a monthly accrual of sick leave.

Sick leave accruals for part-time employees will be proportionate to the number of hours in pay status, in the month to that required for full-time employment. Pay status includes hours worked, time on paid leave and paid holiday.

Vacation (Annual Leave)

Full-time employees accrue vacation leave at the rates specified in WAC 357-31-165(1) or the applicable collective bargaining agreement (CBA). Full-time employees who are in pay status for less than 80 nonovertime hours in a month do not earn a monthly accrual of vacation leave.

Part-time employees accrue vacation leave hours in accordance with WAC 357-31-165(1) or the applicable collective bargaining agreement (CBA) on a pro rata basis. Vacation leave accrual will be proportionate to the number of hours in pay status, in the month to that required for full-time employment.

Pay status includes hours worked, time on paid leave and paid holiday.

As provided in WAC 357-58-175, an employer may authorize a lump-sum accrual of vacation leave or accelerate the vacation leave accrual rate to support the recruitment and/or retention of a candidate or employee for a Washington Management Service position. Vacation leave accrual rates may only be accelerated using the rates established WAC 357-31-165.

Note: Most agencies follow the civil service rules covering leave and holidays for exempt employees even though there is no requirement for them to do so. However, agencies are required to adhere to the applicable RCWs pertaining holidays and leave.

Military Leave

Washington State supports members of the armed forces with 21 days paid military leave per year.

Bereavement Leave

Most employees whose family member or household member dies, or for loss of pregnancy, are entitled to five (5) days of paid bereavement leave. In addition, the employer may approve other available leave types for the purpose of bereavement leave.
