Responsible for developing and coordinating initiatives directed at the appropriate use of antimicrobials within the organization. This includes developing, implementing and maintaining antimicrobial stewardship program, daily patient care monitoring and continuous performance improvement. Responsible for interdisciplinary education regarding antimicrobials and AMP programs.
Pharmacist will double check and continue to review neonatal and pediatric doses of medications and IV fluids to ensure that all weight- based doses are appropriate. This includes the ability to appropriately dose & assess medications that require pharmacokinetic adjustments, as well as quickly assess the literature for atypical medications & their doses Pharmacist should be able to review and evaluate NICU TPN orders for appropriateness and be able to write a NICU TPN if requested to by physician. Pharmacist should also be able to write a pediatric TPN in conjunction with the physician’s specifications (ex: fluid rate) when consulted
QUALIFICATIONS:
Doctor of Pharmacy or Master’s degree required. Pharmacy residency training or Board Certification with added qualifications in pediatrics is preferred.
Dedicated Pediatric Lead pharmacist with the equivalent of > 5 years’ experience providing pharmaceutical care for neonates and/or pediatric patients. Or PGY1 Residency with emphasis on pediatrics within a pediatric hospital Or PGY2 Residency with Pediatric specialty
Board Certified Pediatrics. Maintain a knowledge of NALS in regards to appropriate medication, dose, and administration
Registered pharmacist licensed in the State of Florida.
