Job Description Summary:

Job Profile Summary
Responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care under supervision of the Associate Medical Director.

Job Description:

Location: Supporting our hospice team in the Great Lakes Region.

Shift: PRN One day per week. Orientation is Monday-Friday dayshift (8am-5pm) for the first 2 weeks.

  • Provides medical care for hospice patients. Utilizes assessment, intervention and teaching to maximize patient comfort and to enhance the quality of life to the patient/family unit.

  • Evaluates the health status of selected patients, orders treatment plans and provides education to patients and their families under the supervision of a licensed physician.

  • Performs clinical evaluations and reviews diagnostic studies of patients under the supervision of a licensed physician.

  • Participates in interdisciplinary team conferences of the facility teams, as needed.

  • Interacts with patients and families to manage the psychological, social and family issues regarding hospice and the services rendered.

  • Collaborates and consults with the hospice management team to facilitate problem solving toward meeting patient needs. 

  • Completes all documentation timely and accurately in the patient records on daily visits.

  • Participates in staff meetings, department meetings, in services and other related activities. 

  • Attends educational and developmental seminars and meetings, as deemed appropriate.

  • Provides necessary billing documentation in a timely manner to the hospice Finance department.

  • Performs all other duties as assigned

Knowledge, Skills, and Abilities:

  • Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]

  • Knowledge of electronic medical record.[Required]

  • Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]

  • Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.[Required]

  • Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.[Required]

  • Ability to follow standard precaution guidelines for infection control, including when providing patient care.[Required]

  • Ability to complete AdventHealth Hospice Care Orientation and Training, as necessary.[Required]

Education:

  • Master's of Nursing [Required]

Field of Study:

  • N/A

Work Experience:

  • 3+ years of experience in medical nursing, hospice and/or critical care [Required]

  • 2+ years of experience as an APRN [Required]
