Pay Range: $115,000-$145,000 Per Year
Description:
• Join our team as an Advanced Practice Practitioner specializing in home-based care.
• This full-time role in Brooklyn, NY, involves delivering patient care both in-person and via telehealth.
• You will be responsible for comprehensive patient management, documentation, and ensuring high-quality care. Proficiency in general office technology is required.
Requirements:
• Master's degree in nursing or related science field.
• Active and unrestricted NP/PA license in the state of hire.
• 2 years of direct patient care as a Nurse Practitioner preferred.
• Experience in community-based care is preferred.
• Proficiency in Medicare/Medicaid processes preferred.
• Strong verbal and written communication skills.
• Excellent computer skills, particularly with general office software.
Qualifications:
• 2 years of clinical experience treating complex chronic conditions.
• Experience with high-needs patients and LTSS preferred.
• Strong critical thinking and analytical skills.
• Ability to work in a fast-paced environment.
• Good judgment and impeccable ethics.
• Strong team player with a desire to grow.
