We are seeking an Advanced Practice Provider to join one of the nation s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This opportunity is located at our Stop 11, Family Medicine Practice, at 5908 E. Stop 11 Road Indianapolis, IN 46237.
Highlights
Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Stop 11, Family Care Medicine practice.
Family Medicine Practice inclusive of all age ranges
Single, robust electronic medical record system (EPIC)

Experience
Family Medicine experience as a Nurse Practitioner is preferred
Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.
Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
