Description
Penn Medicine Lancaster General Health Outpatient Internal Medicine, located in Lancaster, Pennsylvania, is searching for a full-time Advanced Practice Provider (CRNP/PA-C) to join our team! Our collaborative care team works to help patients establish goals, manage their health, connect to community/financial resources, and assist with referrals to other services. Were looking to expand our team to welcome a Nurse Practitioner or Physician Assistant. Due to the nature of this role and complexity of patient care, it is preferred that candidates have at least 2 years of experience in primary care.
Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Consults with patients to determine the appropriate course of treatment.
Recommends and orders tests to determine extent of illness or help diagnose condition.
Monitors lab results, medication adherence and medication toxicities.
Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.
Explain and discuss procedures, test results, or prescribed treatment with the patient.
Coordinate work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.
Refers patients to medical specialists when necessary.
Minimum Qualifications:
Current Licensure as a Physician Assistant or Nurse Practitioner (FNP or AGNP)
Valid, unrestricted Federal Drug Enforcement Agency (DEA) number
Participation as a provider in good standing in the Medicare, Medicaid, and Tricare programs and such other third-party-payment programs, including managed care programs in white LGMG participates.
