Job SummaryWe are seeking a dedicated and skilled Advanced Practice Provider (Board Certified Nurse Practitioner or Certified Physician Assistant) to join our healthcare team. The ideal candidate will play a crucial role in delivering high-quality patient care in a post-acute care setting.
Duties
Conduct thorough patient assessments and develop individualized care plans.
Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
Administer treatments and procedures as per established protocols.
Maintain accurate medical records and documentation in compliance with medical terminology standards.
Stay updated on the latest advancements in physiology knowledge and trauma care practices.
Participate in quality improvement initiatives and contribute to the development of best practices within the facility.
Experience
Previous experience in skilled nursing, long term care and rehab
Active New York license as a Nurse Practitioner or Physician Assistant
Job Type: Full-time
Pay: $100,000.00 - $140,000.00 per year + Sign on and quarterly incentives
