We’re hiring an Advanced Practice Provider (NP or PA) - Float who wants a predictable weekday schedule, variety in their workday, and the stability of an established physician-led primary care organization.

You’ll step into an outpatient role focused exclusively on adult primary care, providing coverage across established Jacksonville-area clinics. You’ll manage acute and chronic conditions, preventive care, wellness visits, medications, diagnostics, and referrals with support from a multidisciplinary care team.

What makes you a great fit

• You are a Board Certified Nurse Practitioner or licensed Physician Assistant
• You have at least 2 years of adult primary care experience
• You’re comfortable independently managing acute and chronic conditions
• You value collaborative, team-based outpatient care
• Medicare Advantage experience is preferred
• Spanish proficiency is preferred, but not required

Compensation & Benefits

• Base salary: $130,000–$160,000
• Quarterly bonus opportunity
• Health insurance: Employer pays approximately 80%
• Dental insurance: Employer pays approximately 75%
• Vision insurance: Employer pays approximately 75%

• $3,000 annual CME allowance plus paid CME time
• Malpractice insurance coverage
