We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomesnot rushed visits and volume-driven metrics.
In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.

For experienced APPs, incentives reflect independent panel management, longitudinal patient outcomes, and contribution to population health goals.
For newer or early-career APPs, the compensation model emphasizes stability, structured onboarding, mentorship, and a gradual ramp-up of panel and incentive expectations.
