    Job Title: Advance Practice Provider (Advance Practice Registered Nurse or Physician Assistant) 
     Reports To: TVE Executive Committee
    Pay Status: Per-Diem / Salaried Exempt
     
     
    Position Summary:
    Working under the direct supervision of collaborative physicians, physician site leaders, and a practice manager.  The PA position is responsible for providing and coordinating medical, surgical and other specialty care to patients under the auspices of a supervising physician or the supervising physician’s agent in various settings.  This position will provide patient assessments through patient exams, patient medical history, interpretation of required diagnostic procedures, and developing/implementing treatment and management plans.
     
    Principal Duties & Responsibilities:
     
    1. In collaboration with Physicians of the Company, the Advanced Practice Provider shall provide medical care to include office and inpatient diagnosis, and treatment of patients with primarily vascular surgical and medical ailments.
    2. Maintain office hours determined by the Company to meet the needs of its patients.
    3. Service all patient populations, including Medicare and Medicaid patients.
    4. Communicate with attending physician regarding plan of care and involve physician in clinical decision making outside of the knowledge base or scope of practice of the Advanced Practice Provider.
    5. Educate patients and family members regarding health and illness prevention, medication and treatment instructions, and recommend community resources as needed.
    6. Coordinates necessary tests, consultation, and procedures as indicated or requested by the attending physician.
     
    Required Experience, Education & Licensure:
     
     
    The Advanced Practice Provider shall maintain an unrestricted license to practice as either an Advanced Practice Registered Nurse or a Physician Assistant in the State of Connecticut. The Advanced Practice Provider will also be duly registered to prescribe narcotics with both the DEA and the State of Connecticut.  The Advanced Practice Provider shall obtain and continue to maintain admitting staff privileges at the Hospitals designated by the Company.
