The Position:
Our client is seeking an experienced Nurse Practitioner to provide primary care services in assisted living facilities throughout the Albany region.
Part-Time Option
- 2 – 3 Full Days per Week
- Travel Between Hoosick Falls, NY and Cambridge, NY
OR
Full-Time Option
- 4 – 5 Full Days per Week
- Travel Between Hoosick Falls, NY, Cambridge, NY, and Troy, NY

Requirements:
- Master’s Degree in Nursing from an Accredited Nurse Practitioner Program
- Active New York State Nurse Practitioner License
- Active DEA Registration
- Currently Enrolled and Credentialed with Medicare and Medicaid (Required)
- Ability to Practice Independently (Minimum 3,600 Clinical Practice Hours in Accordance with NYS Requirements)
- Primary Care Experience Required
- Strong Clinical and Communication Skills
Preferred Qualifications:
- Geriatric Experience
- Assisted Living Experience
- Long-Term Care Experience
Responsibilities:
- Monday – Friday Schedule
- No Nights
- No Weekends
- Conduct comprehensive health assessments, physical examinations, and medication reviews
- Develop and implement individualized treatment plans
- Manage chronic diseases and acute medical conditions
- Coordinate care with facility staff, specialists, and family members
- Provide preventive care and health education
- Maintain accurate and timely clinical documentation
- Ensure compliance with Medicare, Medicaid, and regulatory guidelines
- Additional Duties as Required
