Key Responsibilities
- Collaborate with the interdisciplinary team to manage and deliver individualized patient care plans
- Meet critical deadlines for charting and documentation
- Self-manage time, routing, and daily schedule effectively
- Demonstrate proficiency using tablets, mobile devices, and EMR software
- Provide compassionate hospice care to patients and families
Required Qualifications
- Associate degree or higher
- Valid Registered Nurse (RN) license in Pennsylvania (PA)
- 2+ years of Hospice experience
- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR systems
- TruChart EHR
- Valid Driver's License
- Reliable mode of transportation
- Active auto insurance
Schedule & Shift Details
- Monday – Thursday: 5:00 PM – 8:00 AM
- Full-time, direct-hire role with comprehensive benefits
Travel
- Local travel required within the service area
