Key Responsibilities
Collaborate with the interdisciplinary team to manage and deliver individualized patient care plans
Meet critical deadlines for charting and documentation
Self-manage time, routing, and daily schedule effectively
Demonstrate proficiency using tablets, mobile devices, and EMR software
Provide compassionate hospice care to patients and families
Required Qualifications
Associate degree or higher
Valid Registered Nurse (RN) license in Pennsylvania (PA)
2+ years of Hospice experience
1+ years of experience in ANY of the following:
Electronic Medical Record (EMR)
Other EMR systems
TruChart EHR
Valid Driver's License
Reliable mode of transportation
Active auto insurance
Schedule & Shift Details
Monday – Thursday: 5:00 PM – 8:00 AM
