Position Overview


We are seeking a reliable and clinically sound Licensed Vocational Nurse (LVN) to provide after-hours on-call coverage for our Home Health and Hospice patients in Orange County. This role involves telephone triage and occasional in-person visits when urgent patient care needs arise.


Call volume is typically low (0–2 calls per shift). An RN is available for consultation via phone or text should clinical guidance be needed.


This position includes a guaranteed on-call stipend, even if no calls or visits occur.


Schedule


Weekday On-Call Coverage
 Monday – Thursday
 6:00 PM – 8:00 AM
 $360 weekly on-call stipend
 (Friday night coverage is not included in weekday on-call)


Optional Weekend Coverage
 Friday 6:00 PM – Monday 8:00 AM
 $400 weekend on-call stipend


Compensation


On-Call Stipend

• $360 per weekday on-call week

Additional Compensation

• Hourly pay for time spent handling patient phone calls
  
  
• Hourly pay for any in-person visits performed
  
  
• Mileage reimbursement at current IRS mileage rate

Equipment Provided

• Company phone provided for all on-call communications
  
  

Responsibilities

• Respond to after-hours patient or caregiver calls
  
  
• Provide telephone triage and nursing guidance
  
  
• Determine when in-person visits are necessary
  
  
• Perform PRN home visits when urgent issues arise
  
  
• Document patient interactions and visits in Kinnser (WellSky) EMR
  
  
• Communicate patient concerns to the RN on-call as appropriate
  
  
• Maintain patient safety and clinical standards during after-hours coverage
  
  

Qualifications

• Current California LVN license in good standing
  
  
• Strong clinical assessment and communication skills
  
  
• Ability to work independently during after-hours coverage
  
  
• Reliable transportation and valid driver's license
  
  
• Comfortable documenting in electronic medical records
  
  

Additional Information

• Service area: Orange County only. Must be able to visit patients if the need arises while on call.
