About the Role
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our dedicated care team in Pittsburgh. This full-time role is ideal for a nurse who is passionate about providing exceptional end-of-life care, thrives in a supportive interdisciplinary environment, and values meaningful patient relationships.
You will play a key role in delivering high-quality hospice services, ensuring comfort, dignity, and support for patients and their families.
Key Responsibilities
- Provide comprehensive hospice nursing care in accordance with individualized care plans.
- Conduct patient assessments, monitor symptoms, and manage pain and comfort needs.
- Collaborate with physicians, social workers, chaplains, and caregivers to ensure holistic care.
- Document all care activities accurately within the EMR system.
- Educate patients and families on care processes, medications, and expectations.
- Participate in interdisciplinary team meetings and care planning.
- Maintain compliance with all state, federal, and organizational standards.
Required Qualifications
- Associate degree or higher in Nursing.
- Active Registered Nurse (RN) license in Pennsylvania (PA).
- 2+ years of Hospice experience.
- 1+ year of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR systems
- TruChart EHR
