THE POSITION
Located in Hawley, Wayne County, PA a rewarding opportunity for a patient, compassionate, and organized individual who enjoys working with senior citizens and supporting their independence and well-being. Duties include assisting senior day center participants with personal care and daily needs while promoting safety, dignity, and independence. DESCRIPTION OF WORK
This role supports older adults and eligible individuals by providing information, assessments, referrals, and direct assistance across Agency programs. It requires clear communication, attention to detail, and teamwork across various Agency units. As an Aging Case Aide 1, you will perform the following duties:
Initial Contact: Serve as the first point of communication for individuals seeking Agency services
Assessment Work: Conduct interviews, complete intake forms, explain Agency programs, and enter information into Agency databases
Service Coordination: Refer consumers to Care Management staff and coordinate services following Agency procedures
Caseload Maintenance: Manage initial and annual reassessments for the In-Home Meals Program
Daily Living Support: Assist participants with personal care, mobility, and safe transfers
Vital Monitoring: Obtain and record vital signs and assist with rehabilitative activities as directed
Meal Support: Assist with meal preparation, serving, feeding, special diets, and clean-up
Activity Assistance: Assist in recreational and therapeutic activities and supervise participants during program activities
