THE POSITION

Located in Hawley, Wayne County, PA a rewarding opportunity for a patient, compassionate, and organized individual who enjoys working with senior citizens and supporting their independence and well-being. Duties include assisting senior day center participants with personal care and daily needs while promoting safety, dignity, and independence. DESCRIPTION OF WORK

This role supports older adults and eligible individuals by providing information, assessments, referrals, and direct assistance across Agency programs. It requires clear communication, attention to detail, and teamwork across various Agency units. As an Aging Case Aide 1, you will perform the following duties:

  • Initial Contact: Serve as the first point of communication for individuals seeking Agency services

  • Assessment Work: Conduct interviews, complete intake forms, explain Agency programs, and enter information into Agency databases

  • Service Coordination: Refer consumers to Care Management staff and coordinate services following Agency procedures

  • Caseload Maintenance: Manage initial and annual reassessments for the In-Home Meals Program

  • Daily Living Support: Assist participants with personal care, mobility, and safe transfers

  • Vital Monitoring: Obtain and record vital signs and assist with rehabilitative activities as directed

  • Meal Support: Assist with meal preparation, serving, feeding, special diets, and clean-up

  • Activity Assistance: Assist in recreational and therapeutic activities and supervise participants during program activities

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Note: This position provides services for the Wayne County Area Agency on Aging in both the Intake/Assessment/In-Home Meals Unit in Honesdale and the Adult Daily Living Center (ADLC) in Hawley. The incumbent may be assigned to either location based on operational needs, and work schedules. Assignments may change with limited notice.
  • Overnight travel and out-of-county travel for meetings or training may be required.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year of public contact work, which involved obtaining or providing information; or
  • Any equivalent of experience and training.

Other Requirements:
