What You'll Do
As an Aging Services Coordinator, you'll serve as a trusted guide for individuals and families by helping them understand available resources and develop personalized plans that support their goals and independence.
In this role, you will:
- Conduct person-centered assessments by phone and through home visits.
- Develop individualized care plans that reflect each person's goals, strengths, and preferences.
- Coordinate services with healthcare providers, caregivers, family members, and community organizations.
- Connect individuals with local, state, and federal resources including Medicare, Medicaid, SNAP, transportation, housing, caregiver support, and other community programs.
- Provide ongoing follow-up and support for individuals with complex or changing needs.
- Advocate for individuals experiencing barriers to services.
- Maintain accurate documentation and case records using electronic data systems.
- Represent JABA through community outreach, educational presentations, and local events.
- Build strong partnerships with healthcare providers, nonprofit organizations, and community agencies.
- Stay current on aging services, public benefits, and community resources through ongoing professional development.
What We're Looking For
We're seeking someone who combines compassion with professionalism and enjoys helping others navigate complex situations.
Required Qualifications
- High school diploma or GED.
- At least two years of experience in human services, healthcare, social services, case management, customer service, or a related field.
- Strong communication, organization, and problem-solving skills.
- Ability to work independently while managing multiple priorities.
- Comfortable using Microsoft Office and electronic documentation systems.
- Valid Virginia driver's license, reliable transportation, and willingness to travel throughout the service area.
Preferred Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Gerontology, Public Health, Nursing, or a related field.
- Experience working with older adults, individuals with disabilities, or caregivers.
- Knowledge of Medicare, Medicaid, and community resource systems.
- Experience conducting assessments, service coordination, or case management.
- Bilingual language skills are a plus.
