PURPOSE OF POSITION The primary purpose of the Hospice Aide is to provide personal care and homemaker services based on the registered nurses initial and ongoing assessments of the patients personal care needs, ability to perform activities of daily living and supervision of care. RESPONSIBILITIES EXPECTATIONS Assists with personal hygiene, elimination, feeding and mobility according to the patients needs and the nursing orders Provides support for and reinforces the teams teaching of patients caregivers on basic personal care, including turning and repositioning patients, making beds and bathing and showering patients as needed Delivers care in all settings where hospice care is provided, including hospice inpatient facility, private residences, assisted living facilities, nursing facilities or group homes as assigned Communicates with the hospice nurse regarding services provided and significant findings regarding the patients functional status and change in care needs Documents the care provided and the patients response to care Provides homemaker services, including light housekeeping and meal preparation, in the patients home according to the patients needs and the nursing orders Completes and maintains documentation of at least 12 hours of in-service training during each 12-month period Participates with the interdisciplinary team in the development and implementation of the patients and familys plan of care Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community Participates in the Quality Assessment Performance Improvement (QAPI) program, as requested Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow Complies with agency infection control and safety policies Completes annual competency requirements Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Unit Director or designee Protects patient health information according to the HIPPA Privacy Rule and related Aultman policies Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation Supports strategic organizational goals Abides by the Aultman Code of Conduct at all times The above statements reflect the general duties considered necessary to describe the principle functions of the job as identified and shall not be considered as a detailed description of all the work requirements that may be inherent to this position. QUALIFICATIONS STNA certification One (1) year experience in home care or other related healthcare experience preferred Current BLS certification Valid Ohio Drivers License Must have own transportation and provide proof of current liability insurance WORKING CONDITIONS: Hours of operation with shifts as assigned including occasional overtime, on-call or off-shifts (evenings and/or weekends) scheduled as necessary. Lunch and break periods must be coordinated with other staff members to maintain adequate staffing during hospice hours of operation. Subject to frequent interruptions and changes in priority of duties throughout the day.
