QUALIFICATIONS

• Current RN license in the state of Connecticut.
• Experience in home care or pediatric settings preferred.
• Strong knowledge of medical terminology and diagnostic evaluation processes.
• Excellent communication skills to interact with children and their families.

RESPONSIBILITIES

• Coordinate total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventative practices, coordinating services and teaching/training activities.
• Evaluate effectiveness of nursing service to the client and family.
• Consult with the attending physician concerning alterations to care plan; discuss with the appropriate supervisor to make changes as needed.
• Submit clinical notes, progress notes and other documentation outlining services rendered.
• Participate in case conferences; discuss with the appropriate supervisor potential concerns.
• Continually strive to improve nursing care skills by attending in-services and other educational opportunities.
