Job Description

The UW Medical Center-Montlake Pharmacy Department has an outstanding opportunity for an Ambulatory Clinical Pharmacist (Pharmacist 3).

WORK SCHEDULE

  • Full-Time / 40 hours per week
  • Day Shifts

POSITION HIGHLIGHTS

  • Be responsible for providing direct patient care (in-person and telemedicine) in specialty clinics (e.g. transplant, cardiology, anticoagulation, diabetes, rheumatology, dermatology, digestive health and lung disease)
  • Work in conjunction with other clinic providers to co-manage patients via provider referral and collaborative drug therapy agreements
  • In addition to direct patient care responsibilities, the pharmacist will provide drug therapy consults, provide patient/family/staff education, facilitate transition of care, and precept pharmacy students and residents

PRIMARY RESPONSIBILITIES

  • Ensure appropriate drug therapy including dose, drug-interactions, adverse effects, drug allergies, formulary status, and cost effectiveness.
  • Communicate effectively (written and verbally) with inpatient discharging teams and ambulatory transplant teams regarding potential medication related problems and work collaboratively to resolve.
  • Provide patient specific recommendations and guidance for medication administration, titration parameters if needed, and address medication related questions.
  • Provide primary preceptorship of pharmacy residents and student pharmacists
  • Provide education to medical/surgical providers and students and nursing staff
  • Present at educational conferences to multidisciplinary staff, as requested
  • Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requested
  • Participate in collaborative research projects with healthcare professionals, pharmacy residents and student pharmacists
  • Maintain drug therapy monitoring systems for patients

REQUIRED POSITION QUALIFICATIONS

Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

State of Washington Pharmacist License

Compensation, Benefits and Position Details

$151,716.00 annual
