• We work each day to achieve our vision — to provide excellent care to patients and families, with the best staff, in the safest environment.The Staff Nurse in Division of Colorectal Surgery provides high quality, patient centered nursing care to adult surgical patients, including those undergoing colorectal surgical procedures, within the ambulatory setting. This role supports patient care through clinical telephone triage, care coordination, patient education, and collaboration with Advanced Practice Providers (APPs) and the interdisciplinary care team to ensure safe, timely, and effective care.The Staff Nurse systematically and continuously collects and assesses data related to patient health status to develop, execute, and evaluate the plan of care in the ambulatory/outpatient care setting. They provide necessary assessments, administer treatments, and communicate with the multidisciplinary care team. The Staff Nurse provides patient and family education and psychosocial and emotional support. They ensure that care is safe, holistic, efficient, effective, timely and meets the cultural, spiritual, and ethnic needs of each patient and family.The Staff Nurse will be responsible for:
• * Provide clinical telephone triage and nursing support for Division of Colorectal Surgery patients, including medical and pre
• and post operative surgical patients.
• * Support pre operative and post operative Colorectal Surgery patients through ambulatory symptom assessment, care coordination, and patient education.
• * Perform clinical assessment of patient symptoms, determine urgency, and facilitate appropriate disposition and follow up.
• * Providing post-operative wound assessment and care with support from APP as needed.
• * Attending monthly Division staff meetings and other staff meetings as requested.
• * Identify and escalate urgent or complex clinical concerns to the appropriate Advanced Practice Provider.
• * Ability to problem solve, functions as a resource to other members of the multidisciplinary care team and resolves complex issues on behalf of providers and patients.
• * Coordinate care across the Colorectal Surgery practice to ensure continuity, timely follow up, and effective communication with patients, families, and referring providers.
• * Provide patient education related to Colorectal Surgery conditions, surgical preparation and recovery expectations, medication management, diagnostic testing, and follow up care.
• * Accurately document nursing assessments, interventions, and plans of care in the electronic medical record (Epic), using SBAR format when appropriate.
• * Exceptional organizational skills and flexibility to manage multiple tasks and adapt to quickly changing situations.
• * Knowledge of computer applications is necessary to function in an ambulatory/outpatient setting.
• * Maintains and updates clinical knowledge and skills, as they relate to specialty practice and clinical expertise, based on the most up-to-date knowledge, research and current evidence-based practices.
• * Participate in departmental quality improvement and quality management initiatives.
• * Collaborate with physicians, advanced practice providers, nurses, and support staff to support efficient clinic operations and high quality patient care.
• * Perform other duties as assigned by Colorectal Surgical leadership in support of patient care and clinic operations.
• * Responsible for managing the clinic pager to facilitate effective and efficient triaging of patient requests and ensure proper care delivery.
• * Provides clinical support and coverage as needed in the absence of APP colleagues.

  Qualifications

  To qualify you must have:

  Education:

  • Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required for newly licensed nurses

  Experience:

  • 1. Minimum of 2 years of recent (within 12 months) clinical nursing experience in an Ambulatory Clinic/Practice setting required. Experience only in an ambulatory procedural area /emergency room is not sufficient.

  • 2. Nursing experience with telephone triage required, and telephone triage must be the main responsibility of this experience. This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing the severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care.

  • 3. Minimum of 2 years of Surgical, Colorectal, Wound/Stoma and/or Gastroenterology related experience is strongly preferred.
  • 4. Case Management experience preferred.
  • 5. Experience with team-based care is preferred.
  • 6. Other: Membership in professional organizations is recommended and nursing certification in Ambulatory Care or an Ambulatory Specialty area is preferred.
  • 7. Able to work weekend rotation per the MNA contract

  Other:

  • Certification or desire to obtain certification in Ambulatory Care or an ambulatory Specialty area within 2 years is preferred.
