Shift Information: Tu-Fr 730-500 Off Mondays

On Call, Call Back Requirements: None

Weekend Rotation: None

Holiday Requirements: Closed on Thanksgiving, Christmas, and New Year's Day

Floating Requirements: May float to other nearby Sheboygan clinics in the area depending on staffing needs

About Facility: 

General Information:

• Exam rooms: 6-8  
• Census: Average patients/day: Supporting 4 clinicians in total; 2-3 providers in office daily – each see 18-23 patients/day

Minimum Years of Experience: 2 years of clinic experience; family medicine experience helpful

• Will you accept a first-time traveler: Yes  

Patient Types/Common Diagnoses: 

• Peds (experience preferred) to Geriatrics: Comprehensive preventive care, such as annual physical exams and vaccinations; diagnosis and treatment of acute illnesses and chronic diseases

Patient Ratios:N/A  

Required Licensure & Certifications:BLS

Skills required:

• Triaging, medication refills, handle phone calls, In Basket messaging, EKG, injections, able to help with rooming patients when needed

Support within the Department:

• MA
• Providers  
• Front desk staff  

Technology/Equipment: 

• EMR:EPIC - required  

Floating: 

• May float to other nearby Sheboygan clinics in the area depending on staffing needs
• Will know if floating 1 week in advance, or day of depending on call-outs  

Orientation (facility and unit) : 

• Up to 1 week, based on comfort level 

Scheduling:

• Weekend rotation: No weekends
• On call: No call  
• Holiday Expectations: Closed on Thanksgiving, Christmas, and New Year's Day  
• Clinical Interviewing Services participants only : RTO: As part of our program, we approve up to 7 days of RTO, if the traveler is willing to extend their contract to accommodate. Are you comfortable with this: Yes, up to 7 days allowed
