Job Requirements:

• Associate''s Degree in Nursing (Required)

• 2 - 3 years' experience Clinical Nursing Experience in the specialty area for which Navigator position is being hired (ie: orthopedics, cardiac, oncology, etc.) (Required)

• 2 - 3 years' experience Clinical Nursing Experience in home care preferred (Required)

Job Overview:

The Heart Failure Nurse Navigator serves as a clinical coordinator and patient advocate for patients with advanced heart failure, cardiomyopathies, and remote monitoring programs. This role provides comprehensive navigation, education, care coordination, medication monitoring, and longitudinal follow-up for patients enrolled in CardioMEMS, LifeVest, HeartLogic, Barostim, hypertrophic cardiomyopathy (HCM), cardiac amyloidosis, and other cardiomyopathy programs. The navigator collaborates closely with physicians, advanced practice providers, pharmacists, care coordinators, and ancillary team members to optimize evidence-based care, improve patient outcomes, reduce hospitalizations, and enhance patient experience.

Job Responsibilities:

Remote Monitoring Management

• Manage daily surveillance for CardioMEMS, HeartLogic, and other platforms.

• Review alerts, identify trends, and escalate concerns.

• Assess patient symptoms, adherence, vitals, and volume status.

• Coordinate medication changes and follow‑up testing.

• Ensure documentation, billing support, and compliance.

• Identify and onboard appropriate patients.

CardioMEMS Coordination

• Screen eligible patients and provide education.
• Coordinate insurance authorization.
• Monitor PA pressures and communicate concerns.
• Support GDMT optimization using hemodynamic data.
• Track outcomes and quality metrics.

LifeVest & Wearable Monitoring

• Monitor alerts, compliance, and device events.
• Triage arrhythmia or therapy notifications.
• Coordinate follow‑up and provider review.
• Educate patients and plan transitions to ICD evaluation.

Hypertrophic Cardiomyopathy (HCM) Navigation

• Serve as primary nurse navigator.
• Coordinate imaging, genetic testing, and monitoring.
• Support patients on cardiac myosin inhibitors through REMS.
• Manage serial testing, symptom checks, and titration.
• Provide education and participate in multidisciplinary planning.

Amyloidosis & Cardiomyopathy Navigation

• Coordinate diagnostic workup, referrals, and treatment initiation.
• Monitor adherence and response to therapy.
• Educate patients and caregivers.
• Support collaboration across HF, neurology, hematology, genetics, pharmacy, and imaging.

Heart Failure Disease Management

• Assist with GDMT optimization and adherence.
• Conduct follow‑up assessments and transition‑of‑care coordination.
• Identify readmission risks and implement interventions.
• Monitor symptoms, labs, and status changes.
• Participate in quality improvement initiatives.

Patient Education & Advocacy

• Provide individualized HF and remote‑monitoring education.
• Support self‑management and engagement.
• Assist with insurance, medication access, and financial programs.
• Promote shared decision‑making.

Multidisciplinary Collaboration

• Work with HF physicians, cardiologists, APPs, pharmacists, genetic counselors, care coordinators, social workers, and palliative care.
• Participate in case reviews and specialty program meetings.
• Serve as liaison across inpatient, outpatient, procedural, and community teams.
