Join AmeriCorps as a Health Educator with a focus in Health Promotions through Lending Hands for Life at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and HIV-focused outreach through the Lending Hands for Life program. Members gain hands-on experience in community health while making a meaningful impact on underserved communities.

This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.

Main Duties & Responsibilities

• Provide one-on-one health education and self-management support to patients

• Participate in community events, training, and AmeriCorps service activities

• Support Lending Hands for Life HIV programming and re-engagement efforts

• Conduct outreach and engagement for patients requiring additional support

• Document patient interactions and coordinate follow-up care activities

• Facilitate health education groups and recruit participants for programs

Education

Required:

• High school diploma or equivalent.

Skills and Knowledge

Required:

• Strong verbal and written communication skills.
• Ability to work with diverse populations.
• Current CPR certification
• U.S. citizen, national, or lawful permanent resident.
• Ability to meet AmeriCorps background check requirements.

Preferred:

• Experience in community health, social services, or related fields.
• Intermediate Microsoft Office skills.
• Experience with electronic health record systems.
• Bilingual English/Spanish skills.
