Join AmeriCorps as a Social Determinants of Health (SDOH) Health Educator at Erie Family Health Centers. This year-long, full-time service position provides the opportunity to help connect individuals and families to resources that support their health and well-being. This is a hands-on experience in community outreach, resource navigation, benefits enrollment, and patient support while building professional skills through training and service.

This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.

Main Duties & Responsibilities:

  • Screen individuals and families for social drivers of health needs and connect them with appropriate resources.
  • Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs.
  • Assist with enrollment in eligible benefits, programs, and services.
  • Support outreach activities and follow-up with individuals regarding resource needs.
  • Provide digital navigation assistance and help individuals access available services.
  • Complete documentation, data entry, and program tracking activities.

Applications must be submitted by September 13, 2026, to be considered for full-time opportunities. Anticipated start of Monday, September 21st 2026.

Education

Required:

  • High school diploma or equivalent.

Skills and Knowledge

Required:

  • Strong verbal and written communication skills.
  • Current CPR Certification.
  • Ability to work with diverse populations.
  • U.S. citizen, national, or lawful permanent resident.
  • Ability to meet AmeriCorps background check requirements.

Preferred:

  • Experience in community health, social services, or related fields.
  • Intermediate Microsoft Office skills.
  • Experience with electronic health record systems.
  • Bilingual English/Spanish skills.
