Annual Wellness Visit Nurse - LPN or RN (Full Time )

Location: Clarion County

Position Overview

The Medicare Annual Wellness Visit Nurse is responsible for implementing Annual Wellness Visits and Chronic Care Management services at their assigned PHN sites. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or Welcome to Medicare) and Annual Wellness Visits both initial and subsequent. Chronic Care Management services consist of monthly activities designed to intensively manage chronic conditions of qualified patients and to coordinate all care that patient receives. Working in close collaboration with other departments and programs, the Medicare Annual Wellness Visit Nurse also builds individual and community capacity by increasing knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

Compensation

Position starts at $20.00 (LPN) per hour, or $26.00 (RN) however, PHN recognizes experience, education and site location when determining wage.

Hours to not exceed 40 per week

NO WEEKENDS

Qualifications

Medical Assistant (MA):graduate of an approved school Medical Assistant (certification preferred) and/or two years experience in a clinical setting

Licensed Practical Nurse(LPN): graduate of Licensed Practical Nurse program and licensed or eligible to take state boards.

Infection Control and strong clinical knowledge

Administering Medication

Previous venipuncture and injection experience preferred

Previous experience in calling prescriptions under a providers direction

Computer proficient and EMR experience
