Advanced Practice - Cardiology


Job Details:

  • Cardiac Rehab Coordinator role in an outpatient rehabilitation clinic setting located offsite in a Medical Office Building

  • No hospital bedside care, procedures, or cath lab involvement

  • Schedule: Monday through Friday, 8:00 AM to 5:00 PM with a one-hour break; last patient seen at 4:30 PM

  • Highest patient volume on Monday, Wednesday, and Thursday (approximately 16 patients per day); new patient intakes on Tuesday and Friday (approximately 2 to 4 patients)

  • Bilingual English/Spanish proficiency is highly preferred

  • EMR system: Scottcare Versacare Software

  • Provide direct supervision for patients receiving cardiac or pulmonary rehabilitation and immediately respond to any adverse events

  • Work closely with Medical Directors to evaluate need, develop plan of care, and implement care for cardiac and pulmonary rehabilitation patients

  • Coordinate patient care in collaboration with a multidisciplinary team including exercise physiologists, rehabilitation technicians, and licensed Respiratory Therapists

  • Coordinate all aspects of multidisciplinary education and training required for cardiac rehabilitation and ensure psycho-social and educational needs are met

  • Set up, monitor, and document cardiac and pulmonary rehabilitation services ensuring all regulatory components are met

  • Perform medical assessments including history and physical assessments and consult with physicians regarding findings or changes in patient condition

  • Ensure compliance with all CMS requirements concerning the provision of cardiac and pulmonary rehabilitation

  • Develop individualized exercise and treatment plans for review and approval by the medical director

  • Diagnose medical and nursing conditions, plan and implement interventions, and evaluate patient response to therapy under the direction of and in collaboration with the supervising or attending physician

  • Must be able to implement ACLS protocol when necessary and utilize cardiac telemetry monitoring

  • Minimum of two years of Cardiac Unit or Telemetry experience required

  • Master's of Science Degree or Advanced Nurse Practitioner certification required

  • Current RN licensure and recognition as a Family or Acute Care Nurse Practitioner required

  • BLS and ACLS certification required
