NeuroRestorative a part of the Sevita family provides rehabilitation services for people of all ages with brain spinal cord and medically complex injuries illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members, and Veterans.

Provide on-site and alternative additional educational services to adolescent program participants in a cognitive retraining and educational setting following their rehabilitation program.

Essential Job Functions

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:

• Plans, schedules, and provides weekly early educational in-home services for assigned caseload of infant-toddlers with special needs. • Administers and documents all program assessments and updates accurately meeting designated timelines. • Maintains confidential client records and files, and keeps all child and family information confidential. • Designs, implements, and adapts activities for language, cognitive, fine, and gross motor, sensory, self-help, and social-emotional development for each individual child utilizing best practices. • Models effective adult-child interactions and provides written information on parenting and child development as needed. • Provides parents with written weekly activities and effective coaching. • Logs all phone and face-to-face contacts with the family and their Infant Service Coordinator. • Completes and submits all required program and client paperwork accurately and in a timely manner. • Protects children and family rights and fulfills the responsibilities of a mandated reporter of suspected child abuse or neglect. • Maintains personal calendar, meets timelines, completes tasks, and allows flexibility in scheduling so as to accompany the Coordinator on initial visits. • Meets parents needs for scheduling home visits, i.e., early evening appointments may be necessary. • Monitors documents and shares the childs progress with the family, Coordinator, Infant Service Coordinator, and immediate supervisor. • Communicates and collaborates effectively with families and all professionals verbally and in writing. • Assists families, Infant Service Coordinators, and SELPA districts with program transitions at the childs 3rd birthday. • Attends SELPA planning meetings and IEPs bi-monthly, staff meetings, and actively participates in additional professional development opportunities. • Abides by all program policies and other Creative Home Programs Mentor guidelines as set forth in the TeacherEmployee Handbook and any amendments presented hereafter. • Performs other related duties and activities as required.

Minimum Knowledge and Skills Required by the Job

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:

Education and Experience:

• Bachelors Degree in Education • Two years of experience working with children in the applicable age group and their parents

Certificates, Licenses, and Registrations:

• Therapy or Nursing License preferred • Special Education certification preferred • Current drivers license, car registration, and auto insurance if providing transportation for individuals receiving services

• Travel required as needed • Physical Requirements: Medium Work. Exerting up to 50 pounds of force occasionally and/or up to 30 pounds of force frequently and/or up to 10 pounds of force constantly to move objects.
