A full-time Assessor position is now available with Liberty Healthcare. This role involves administering structured assessments to help determine individuals’ eligibility for home and community-based services that support independence and wellbeing.
This is a field-based role which will allow you to primarily work remotely from home and require you to travel to client residences throughout the region.
Your daily work will involve
There is some flexibility in the work schedule, which is primarily Monday through Friday during daytime hours.

As a new hire you will receive formal training in industry leading person-centered assessment tools, and over time you will gain valuable resume building skills and experience.
What we’re looking for
This role comes with a high degree of autonomy so you must be very organized and self-motivated.
Since you will be required to travel throughout the region, you must have your own insured vehicle.
You have relevant work experience if you’ve worked as a Direct Support Professional, Case Manager, QIDP, Social Services Intake or Admissions Specialist, Service Coordinator, Client Advocate, or Caseworker.
