We are looking for an Assistant Program Manager/Care Coordinator to help with our vision:Purpose-SC is hiring Full Time Assistant Program Manager/Care Coordinator to provide assistance to our programs located in our Greenville SC area.Purpose-SC provides meaningful and rewarding employment opportunities supporting children and adults with intellectual and developmental disabilities. We provide residential, educational, and recreational programs to support and improve the lives of those we serve in a caring and compassionate team environment.RESPONSIBILITES:Directly supervises other team members within the residential department.Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.Responsibilities include training team members; planning, assigning, and directing work; appraising performance; rewarding and disciplining team members; addressing complaints and resolving problems.On Call Schedule as needed.The Care Coordinator shares a caseload of no more than 10 individuals: Develops an individualized Plan of Care (POC) based on the identified strengths and needs, including a comprehensive 24-hours Crisis Plan: Monitors the provision and quality of services provided to the individual through the Team and is the liaison when new services/resources need to be sought or developed.Provides or secures support and crisis/emergency services to the individual through face-to-face contact, phone contact and availability by on-call system.Completes all the necessary paperwork in a strength-based manner per South Carolina and BBH requirements.Collaborates with other necessary individuals the individual and family may have contact with, such as Case Managers, Probation Officers, Judges, Provides written and verbal information related to the individual's behavioral and mental health needs and progress.Other duties as assignedQUALIFICATIONS:A bachelor's degree required, or related field or equivalent work experienceA minimum of three (3) years progressive management experience working with individuals with mental illness and/or developmental disabilities.Have demonstrated leadership ability: the ability to integrate the vision of the organization with all supportive services and personnel.Meet all licensing requirements set forth by the respective state of operation;Valid state driver's license in good standingAccess to a registered vehicle with proof of insurance if providing transportation for individuals receiving services.Successful clearance of local and state background checks, post-offer drug screening and physicalCapable of successfully performing physical behavior interventionsPatience, excellent communication, and basic computer skillsAbility to work without constant supervision.Understanding of all agencies Standards and Directives, also outside licensing regulations and policies.
