Position Summary: Provides individual, group, and family counseling services to behavioral health patients to support positive treatment outcomes. Conducts behavioral health assessments to develop effective treatment plans specific to each patient. Facilitates and participates in clinical team meetings to monitor and assess patient progress and determine next steps to optimize patient treatment outcomes. Collaborates with patients, their families, and other stakeholders to determine barriers to treatment success and how best to address patient challenges. Works under clinical oversight of an independently licensed professional to conduct behavioral health services.

Critical Tasks:

  • Conducts comprehensive and annual assessments with patients to determine need for services.

  • In collaboration with patients, guardians, advocates, ancillary service providers and/or patient identified supports, complete individualized service plans based on the comprehensive assessment.

  • Provide services in the office, home, community and other facilities as needed.

  • Provide individual, family and group counseling services to patients.

  • Respond to crisis situations, providing immediate support and guidance to patients in distress.

  • Manage their caseload and use available resources to ensure positive treatment outcomes.

  • Maintain accurate and timely medical records, including progress notes, assessments and service plans in accordance with GRHC, state and federal requirements..

  • Educate patients about mental health and wellness, and advocate for their needs within the healthcare system.

  • Coordinate regularly with individual patient clinical team and communicates as appropriate with patient referral sources.

  • Stay current with best practices in counseling and complete all required training.

  • Adhere to GRHC adopted ethical and professional standard, maintaining confidentiality and building trust with patients.

  • Meet regularly with their immediate supervisor for supervision

  • Provide case management duties as needed.

  • Follow all state, federal and tribal laws as well as GRHC policies and procedures.

  • Demonstrate the ability to clearly communicate, both orally and in writing, while performing all essential functions.

  • Work with all ages and populations of patients.

  • Work with general mental health issues as well as substance use, grief and loss, trauma and crisis interventions.

  • Work with all GRHC staff and GRIC departments in a respectful and collaborative manner.

  • Performs special projects and duties as assigned.

Required Qualifications:

  • Bachelor's degree in Social Work or a closely related field of study from an accredited university or college required; Master's Degree preferred

  • 6 months of experience in social work or a related field required.

  • State of AZ Driver's License with no significant violations during the last 5 years.

  • Arizona Department of Public Safety Fingerprint Clearance Card required.

  • Current and continuing CPR/FA and/or BLS training and certifications required.

  • Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
