Overview
The Associate Director, Operations - Transitions of Care (TOC) and High Risk Populations (HRP) is responsible for operational leadership, coordination, and support of Central Health's TOC and HRP service lines and programs. This role partners closely with clinical, nursing, operational, and cross-functional leaders to support program development, service line implementation, workflow standardization, performance monitoring, and operational effectiveness.
The Associate Director supports the Director of Operations - TOC and HRP in developing and sustaining high quality service models, standardized processes, and operational efficiencies. This role provides direct oversight, coaching, and development to assigned team members and supports effective coordination and execution of service line priorities across the organization.
