Job Description:

• This project supports the urgent need to expand intake and placement operations for families transitioning out of the Row and into the shelter system. In alignment with the Agency's goal of completing all transitions by January, operations must accommodate up to 60 families per day and operate over extended hours to meet the accelerated timeline.
• The Outreach Specialist will support a smooth and efficient transition process by providing direct assistance to families during their relocation.
• Responsibilities include assessing family needs, gathering required documentation, and identifying appropriate shelter placements based on family composition, school enrollment, medical needs, and other individual considerations.
• The specialist will coordinate closely with Client teams and contracted service providers to ensure families are transitioned safely, efficiently, and with minimal disruption. Additional responsibilities include maintaining accurate records, updating case information, and monitoring the transfer process.

• Provide direct support to families throughout the relocation and shelter placement process.
• Assess family needs and collect required documentation.
• Identify appropriate shelter placements based on family composition, school enrollment, medical needs, and other special considerations.
• Coordinate with Client teams and contracted providers to facilitate safe and efficient transitions.
• Maintain accurate records and assist with case updates.
• Monitor and track family transfer activities.
• Handle sensitive and confidential information with discretion.
• Work effectively in a fast-paced, high-volume environment while meeting critical deadlines.
