Job Summary:
As part of the multidisciplinary VICC Community Outreach and Engagement population health navigation program, the Assoc Public Health Manager for Cancer Screening provides patient centered, non-clinical navigation services to community members in need of cancer screening, including individuals who are uninsured or underinsured. This position works with community partners to provide education and support around cancer screening and local resources to encourage patient action. Navigation services include individualized assistance to help overcome health care system barriers and facilitate timely access to quality medical care.

Preferred Skills and Experience
Public Health background
Experience facilitating small group discussions, workshops, or educational sessions.
Proven 1:1 or small group health coaching experience with the ability to build trust and support behavior change.
Clinical experience in a healthcare setting.
Experience working in a community clinic or community health environment.
Demonstrated ability to engage and support diverse patient populations.
Strong coaching, communication, and relationship-building skills.
KEY RESPONSIBILITIES
Provide programmatic support as part of the VICC Community Outreach and Engagement team's population health navigation program.
Assist community members in understanding local resources available, including educating patients about cancer screening guidelines and options.
Maintains awareness related to community resources and assists in identifying individuals in need of screening to address barriers by connecting patients to resources.
Supports program leadership with various programmatic and administrative responsibilities.
In the course of performing the duties, integrates with partner organizations, including attendance at relevant meetings and outreach events.
Assist with follow-up of individuals navigated to support action and continued screening.
Maintain up-to-date, complete, and accurate documentation of navigation interactions and track programmatic activities, reporting quarterly to demonstrate outcomes and performance improvement activities.
Assist in the development and implementation of patient and community outreach and education programs as appropriate.
Provides support for the creation, coordination and execution of the program.
Supports program leadership with various programmatic and administrative responsibilities.
Maintains awareness related to research project activities.
