Overview of Asthma Program:
Take a deep breath. For asthmatics, the simple act of breathing can be a struggle. For Bronx residents, asthma is a familiar foe. In the Bronx, Hunts Point has the second highest rate of asthma in the country. Asthma prevalence in the Bronx is 6.2%. Asthmatics in our area have high rates of hospitalization and emergency department visits.
 
Position Summary:
The AsthmaCommunity Health Worker (ACHW) will join our team in order to support comprehensive asthma self-management and education services. The ACHW will do outreach, home visits, and provide preventive and education services to individuals in the Bronx community that are impacted by Asthma and/or dually diagnosed chronic conditions that include Asthma.
 
Minimum Qualifications:
  • High School/GED required with 2 years of related work experience in community health, health education, outreach, home visits, etc. Associates/Bachelors Degree is preferred.
  • Bilingual English/Spanish is required.
  • Effective written and verbal communication skills.
  • Ability to relate well to a diverse patient population, health care providers, clinic support staff, and peers.
  • Proficient with Microsoft Word and Excel- Previous experience with Electronic Health Records is highly desired.

Key Duties:
  • Identifies high risk patients and recruits and schedules them for home-based or clinic-based services.
  • Conducts home visits and/or clinic-based visits in accordance with NAEPP guidelines, ASME toolbox, and environmental assessments.
  • Develops an individualized Asthma Action Plan with each patient.
  • Demonstrates medication use and adherence to patients including "teach backs" to demonstrate understanding
  • Attends meetings, team conferences, trainings, and participates in QI/QA initiatives.
  • Identifies high risk patients using Electronic Medical Record and utilization data to identify patients who may benefit from this service.
  • Conducts outreach in communities with high asthma burden and refers them to NAEPP guidelines. 

Compensation: $46,000/annum based on 40 regular hours per week

Work Environment Type, Location, and Schedule:
Applicants must be comfortable working onsite, based out of the Bronx and have availability to commit to a schedule of 40 hours per week, working Monday through Friday 8am-5pm/9am-6pm. Occasional Saturday shifts as needed, with a day off during the weekdays.
